Make a Notification Using Your Own Form

This form is for organisations and entities covered by the Reportable Conduct Scheme to make a notification about an allegation of child-related harm, where they have their own form for recording the reporting allegations.

Your form should include all the details of the allegation, the person subject to allegations (if known), and the alleged victims. If it does not, complete the Notification Form here.
