This form is for organisations and entities covered by the Reportable Conduct Scheme to make a notification about an allegation of child-related harm.

If your organisation has its own form for recording details of allegations, you can submit your own form here. Your form should include all the details of the allegation, the person subject to allegations (if known), and the alleged victims. If it does not, complete the form below.
